Dear 1040 Client:
Since 2008 we have offered client's the option of "Going Green" and receiving their tax return in PDF format on a CD-ROM. However, if they choose, they may continue to receive their return in the traditional bound paper format. Please indicate below which format you prefer to receive your return. Please note that if you do not select a format, you will automatically receive your tax return in the PDF format.
In addition, we are also asking our client's whether or not you they want to continue to receive the Client Organizer. If you do, please indicate below whether you prefer to receive it in the traditional paper format or in electronic format via secure e-mail. If you no longer want to receive the Client Organizer, please indicate accordingly.
After you have answered the above questions, please include this letter with your tax preparation documents that you provide us to prepare your tax return. You can also return this form via fax or email to our office as soon as possible.
